BioHarmony score
Noopept is a Russian over-the-counter cognitive drug taken at 10 mg twice daily, reported to raise MMSE scores in vascular and post-stroke cognitive impairment over eight weeks. No placebo-controlled or Western trial exists, the longest human study ran 56 days, and it is an unapproved drug in the United States.

Noopept is worth trying if you understand exactly what you are buying: a cheap, fast-acting, mechanistically coherent compound with an evidence base thin enough that one well-run placebo trial could move it in either direction. The felt effect is real for roughly half of people and subtle even then. The safety record for noopept is unalarming in absolute terms, with no published case report of severe harm and two decades of over-the-counter use in one country. It has one specific hole, which is blood pressure. The practical risk sits in the supply chain more than the molecule. There is no lawful US channel, no dose verification, and sale for human consumption is prohibited in the UK under the 2016 Psychoactive Substances Act. Treat it as a personal experiment with a defined course and a defined stopping rule, not as a staple. Supply is also contracting. Nootropics Depot has discontinued its noopept products outright and CosmicNootropic has moved its listing to a liquidation sale, so the tested-capsule end of the market is thinner than it was, and vendor due diligence is now the largest single variable in whether a course is worth running.
✅ Best for: People who have already dialled in sleep, training and nutrition and want a low-cost cognitive experiment with a defined course length. Readers who value a felt effect they can evaluate within an hour rather than a compound they have to take on faith for three months. Anyone comparing the racetam family who wants the option that works at 20 mg rather than grams. People with vascular or post-traumatic cognitive impairment who are discussing options with a clinician, since that is the only population noopept has actually been studied in. Experimenters willing to buy a tested capsule rather than bulk powder and to track blood pressure while they run it.
❌ Avoid if: You have hypertension or take antihypertensives, since a blood-pressure rise is the one reproducible objective finding and the manufacturer specifically warns against that combination. You are pregnant, breastfeeding, under 18, or have severe hepatic or renal impairment, all of which the Russian label rules out. You are prone to emotional blunting on serotonergic or dopaminergic agents, because flattened affect and lost libido are the distinctive negative reports here. You want an evidence base rather than a mechanism, since no placebo-controlled trial exists anywhere. You are unwilling to verify a vendor, because a labelled milligram figure in this category is frequently wrong.
