Noopept or piracetam: which is the better starting point?
Piracetam has the better evidence base and noopept has the stronger felt effect at a far smaller dose. Piracetam runs to grams per day with decades of Western study behind it, while noopept works at 20 mg and rests on two unblinded Russian trials. Noopept is not technically a racetam, though it was designed inside that lineage. If the priority is a defensible evidence base, start with piracetam. If the priority is noticing something, noopept is the more likely candidate.
What could change Noopept (Omberacetam)'s score?

- Ostrovskaya et al. 2002, Eksperimental'naia i klinicheskaia farmakologiia: the original novel nootropic and neuroprotective agent noopept. The foundational 2002 pharmacology review from the lab that developed noopept. It reports that noopept exceeds piracetam by orders of magnitude on effective dose in animals and covers a broader range of memory-related activity in animal models, plus anxiolytic, antioxidant, anti-inflammatory and neuroprotective effects. Same institute holds the compound's pharmacology patents, so read the breadth claim with that in mind.
- Ostrovskaya et al. 2008, Bulletin of Experimental Biology and Medicine: noopept stimulates the expression of NGF and BDNF in rat hippocampus. The single most cited mechanism study. Acute dosing raised hippocampal NGF and BDNF messenger RNA in rats, and a 28-day course potentiated the neurotrophic effect rather than producing tolerance. This is a rodent finding and has never been reproduced in a human tissue or imaging endpoint.
- Neznamov and Teleshova 2009, Neuroscience and Behavioral Physiology: comparative studies of noopept and piracetam in mild cognitive disorders of vascular and traumatic origin. The other identifiable human trial, an unblinded comparison against piracetam over 56 days. Secondary sources report noopept at 20 mg daily, MMSE movement in the noopept arm, and blood pressure rises in a minority of patients, but PubMed carries no abstract and the full text could not be opened, so none of those figures is confirmed against the paper itself.
- Amelin et al. 2011, Zhurnal Nevrologii i Psikhiatrii imeni S.S. Korsakova: noopept in the treatment of mild cognitive impairment in patients with stroke. The strongest human trial available. An open prospective study of 60 stroke patients reported that noopept at 20 mg daily for two months improves cognitive function and has a high level of safety. Open-label with no placebo arm, run inside the same Russian research lineage that developed the compound.
- Ostrovskaya et al. 2014, Journal of Biomedical Science: neuroprotective effect of noopept on an Alzheimer-related cellular model, apoptosis and tau hyperphosphorylation. Cell-model study in PC12 cells exposed to amyloid beta toxicity. Noopept reduced reactive oxygen species, calcium overload and tau phosphorylation at serine 396, and restored mitochondrial membrane potential. This is the clearest neuroprotection evidence in existence for the compound, and it is in a dish.
- Cohen et al. 2020, Neurology Clinical Practice: five unapproved drugs found in cognitive enhancement supplements. The definitive purity study for this category. Among products declaring a quantity, 9 of 12 declared amounts were inaccurate, ranging from 0 to 135 percent of label, and consumers could be exposed to up to four times pharmaceutical dosing. Maximum omberacetam content measured was about 40 mg against a typical 10 mg dose.
- Noopept reduces the postischemic functional and metabolic disorders in the brain of rats with different sensitivity to hypoxia, Bulletin of Experimental Biology and Medicine 2009. Rat bilateral carotid ligation model. Seven days of dosing at 0.5 mg per kg improved survival and behaviour in both hypoxia-sensitive and hypoxia-resistant animals and lowered lipid peroxidation products. This is the animal study that sits closest to the vascular population the human trials enrolled.
- An experimental study of the anti-inflammatory action of noopept and its effect on the level of cytokines, Eksperimental'naia i klinicheskaia farmakologiia 2012. Rodent inflammation study using carrageenan and lipopolysaccharide models. Noopept at 5 mg per kg cut interleukin-6 by roughly 1.8 times and halved tumour necrosis factor alpha against controls. Animal-only, and no human inflammatory marker has been reported.
- Cognitive enhancer noopept activates transcription factor HIF-1, Doklady Biochemistry and Biophysics 2020. Cell study in a neuroblastoma line showing increased HIF-1 DNA-binding activity through prolyl hydroxylase inhibition. This is the mechanism most often invoked to explain the ischemia results, and it has been replicated in a second cell line by the same group.
- Molecular mechanism underlying the action of substituted pro-glycine dipeptide noopept, Acta Naturae 2016. A transcription-factor screen across nine candidates showing that noopept selectively activates HIF-1 and docks to prolyl hydroxylase 2. It corroborates the later neuroblastoma study in a different reporter system rather than adding an independent mechanism, and an earlier description of this paper as a synthesis review was wrong.
- Effect of nootropic dipeptide noopept on CA1 pyramidal neurons involves alpha-7 acetylcholine receptors on interneurons in rat hippocampal slices, Neuroscience Letters 2022. A 2022 slice electrophysiology study showing that noopept raises inhibitory currents onto CA1 pyramidal neurons through alpha-7 nicotinic receptors on interneurons, an effect blocked by two selective antagonists. This is the cholinergic link behind the community habit of pairing noopept with a choline source.
- Pharmacokinetics of noopept and its active metabolite cycloprolyl glycine in rats, Biomeditsinskaia khimiia 2018. The rat study behind the prodrug story. Cycloprolylglycine differs from the parent compound on pharmacokinetic parameters while keeping a similar pharmacological profile. No human study has confirmed that this conversion happens in people at the doses actually taken.
- Fedorova, Us and Ostrovskaya 2007, Neurochemical Journal: antioxidant effect of the nootropic dipeptide noopept on iron-induced chemiluminescence of human serum lipoproteins in vitro. In-vitro antioxidant study in human serum lipoproteins, cited widely for a specific multiple of the chemiluminescence latent period. The title and authorship are confirmed, the abstract text is not accessible, so the reported magnitude should be treated as unconfirmed rather than measured.
- Giurgea 1972, Actual Pharmacol (Paris): pharmacology of integrative activity of the brain, an attempt at a nootropic concept in psychopharmacology. The paper that named the nootropic category while developing piracetam, the compound noopept was explicitly designed to improve on. It is the historical starting point for the whole class. Piracetam is the parent compound of this lineage rather than the subject of this report, and the paper is conceptual rather than a trial.
- FDA warning letter to Peak Nootropics LLC, 2019: unapproved new drugs sold as dietary supplements. The regulatory record for US status, issued in 2019. The agency named noopept among unapproved and misbranded drug ingredients being sold as supplements, which is the reason there is no lawful US retail channel for it today.
- Russian instructions for use, Noopept tablets, 2026 translation: dosing, course length and contraindications. Translation of the Russian package insert, the only formal dosing document that exists for this compound and the source every 2026 vendor page paraphrases. It sets 10 mg twice daily with a 30 mg ceiling, courses of 1.5 to 3 months, and contraindicates pregnancy, breastfeeding, age under 18, and severe hepatic or renal impairment.
