What Does This Health Assessment Quiz Measure?
The Outliyr Health Assessment Quiz evaluates 186 clinical symptom questions across 12 body systems using a 4-point frequency scale (Rare, Occasional, Often, Frequent). Based on functional clinical assessment methodology used by integrative health practitioners, this body systems assessment identifies patterns of biological stress that standard bloodwork misses. Your answers generate a composite picture of how your body is actually performing right now. Not based on a single lab marker. Based on how you feel and function across every major system.
Each domain targets a different physiological system. Here’s what the assessment evaluates:
- Gastrointestinal: Digestion quality, bloating, food sensitivities, and gut inflammation markers
- Liver & Gallbladder: Detox capacity, bile flow, and fat digestion efficiency
- Endocrine: Thyroid and adrenal function, hormone balance, and stress resilience
- Glucose Regulation: Blood sugar stability, energy crashes, and insulin sensitivity patterns
- Cardiovascular: Heart health indicators, circulation quality, and blood pressure signals
- Mood: Emotional resilience, neurotransmitter balance, and mental health markers
- Immune: Immune defense strength, inflammation frequency, and recovery speed
- Urological: Kidney function, hydration status, and urinary tract health
- Musculoskeletal: Joint health, muscle recovery, flexibility, and structural integrity
- Nervous System: Brain function, cognitive clarity, sleep quality, and neurological resilience
- Men’s Health: Testosterone markers, prostate health, and male-specific vitality indicators
- Women’s Health: Hormonal cycle patterns, reproductive health, and female-specific stress markers
How Is Your Biological Stress Score Calculated?
Each question uses a 4-point frequency scale: 0 (Rare), 1 (Occasional), 4 (Often), and 8 (Frequent). Higher scores in a domain mean more biological stress in that system. The assessment totals your responses per domain and converts them to a percentage, then classifies each into a stress band: Low (green, under 30%), Moderate (yellow, 30-60%), or High (red, over 60%).
Your overall biological stress score combines all 12 domains into a single percentage. This gets compared against age-group benchmarks so you can see how you stack up. The Energy Index inverts the stress score to show your available energy capacity on a 0-100 scale. A higher Energy Index means less biological drag on your system.
The bio-age estimate uses the relationship between your stress load and your chronological age to approximate your functional biological age. If your stress profile is lower than average for your age group, your bio-age comes in younger. If it’s higher, your bio-age runs older.
What Results Will You Get?
Your results include a bio-age estimate comparing your biological age to your chronological age, an Energy Index score from 0 to 100 with an age-group percentile ranking, and a Stress Band classification (Low, Moderate, or High). You’ll also receive an Energy Archetype that maps your health profile to a personality-style pattern, plus a Primary Focus Area identifying which body system needs attention first. Every result comes with personalized next steps you can act on immediately.
Here’s what you’ll walk away with:
- Bio-Age Estimate: biological vs. chronological age comparison
- Energy Index: 0-100 score with age-group percentile
- Stress Band: Low, Moderate, or High classification
- Energy Archetype: your personality-style health profile
- Primary Focus Area: which system needs attention first
- Personalized Next Steps: specific actions based on your scores
- BioSystems Breakdown: color-coded scores across all 12 domains
How Does the Health Assessment Quiz Work?
The process takes about 3 minutes from start to finish:
- Select your biological sex. The assessment adapts questions based on male or female physiology. Women see reproductive health questions. Men see prostate and testosterone markers.
- Answer the symptom questions. Rate each symptom based on your typical experience over the past 4 weeks. Skip anything that doesn’t apply. Skipped questions don’t count against you.
- Get your results instantly. No waiting, no email gate. Your bio-age, Energy Index, stress band, archetype, and full BioSystems Breakdown appear immediately.
Who Is This Assessment For?
This health assessment quiz is for anyone who wants a real baseline of how their body is performing across all major systems. If you’re dealing with unexplained fatigue, brain fog, or digestive issues that haven’t shown up on standard labs, this body systems assessment surfaces the patterns your doctor isn’t testing for. Biohackers use it to find blind spots in their stack. Health-conscious people use it to get a clear starting point before investing in supplements or protocols. It takes about 3 minutes, it’s completely free, and you don’t need an account or email to get your results.
Frequently Asked Questions
Is this a medical diagnosis?
No. This assessment is for educational and informational purposes only. It identifies patterns of biological stress based on symptom frequency, not clinical lab work. It’s a compass, not a diagnosis. Always consult a qualified healthcare provider before making changes to your health regimen.
How long does it take?
About 3 minutes for most people. The quiz adapts based on your biological sex, so men and women see slightly different question sets. You can skip questions you’re unsure about without affecting your score.
Do I need to create an account or enter my email?
No. You get your full results immediately without creating an account or providing an email address. Your answers are saved locally in your browser so you can retake the assessment later and track changes.
How often should I retake the assessment?
Every 4 to 8 weeks. That’s enough time for lifestyle changes, new supplements, or protocol adjustments to show up in your symptom patterns. The assessment saves your previous answers so you can compare results over time.
What’s the difference between bio-age and chronological age?
Chronological age is how many years you’ve been alive. Bio-age reflects how your body is actually functioning relative to your age group. Someone who’s 40 with low biological stress might have a bio-age of 35. Someone who’s 30 with high stress across multiple systems might show a bio-age of 37. The gap between the two tells you whether your lifestyle is accelerating or decelerating your aging.
