A week at a longevity retreat can cost more than a used car. Some of them are worth every dollar. Others charge $2,000 per night for yoga classes and a view you could find on Google Earth.

The difference isn’t marketing. It’s whether you leave with a protocol that changes the next 12 months of your life or just a nice memory.

I’ve spent a decade testing the same modalities these longevity retreats offer: red light therapy, PEMF, fasting, HRV training. So I ran the numbers on every major program to answer one question: which ones deliver value you can’t replicate on your own?

Here’s what I found.

What separates a genuine longevity program from an expensive wellness vacation comes down to three things:

The market has exploded since 2023, with programs ranging from $340/night at medically supervised fasting centers to $5,000/night at elite diagnostic retreats.

Canyon Ranch’s LONGEVITY8 program in Tucson is the most comprehensive medical longevity retreat in the US. Four days, $20,000, 200+ biomarkers, 18+ one-on-one expert consultations, and 15+ diagnostic tests including:

  • DEXA body composition
  • VO2 max testing
  • Continuous glucose monitoring (CGM)
  • Carotid ultrasound
  • Full lab panels

A medical board convenes after your departure to review all results. You get five virtual follow-ups. Multiple 2026 dates are already sold out.

Next Health Executive Physical

Next Health isn’t technically a retreat. It’s a one-day executive physical across multiple LA locations and Manhattan. Their protocol covers 1,000+ markers including Galleri multi-cancer detection, full-body MRI, and TruAge biological age testing.

Pricing starts at a $200 consult and scales based on your selected panel. Best for people who want diagnostic depth without committing to a multi-day stay.

Pritikin Longevity Center

Pritikin in Miami offers something no other retreat can: 50+ years of published research and 100+ peer-reviewed studies supporting their protocol. Pritikin is backed by 100+ peer-reviewed studies across 50+ years. At $4,998 to $5,995 per week, it’s the most affordable medical retreat on this list.

The program focuses on lifestyle medicine backed by physician oversight. For someone with metabolic syndrome, hypertension, or diabetes, the ROI per dollar likely beats everything else here.

Best Fasting & Detox Retreats

Fasting retreats have the strongest published evidence of any category.

I’ve done extended fasts myself. The supervision matters.

Water-only fasting under medical supervision has documented outcomes for hypertension, autoimmune conditions, and metabolic health that most luxury retreats can’t match.

TrueNorth Health Center

TrueNorth in Santa Rosa, California is the world’s leading medically supervised water-only fasting facility. In a 2001 study published in the Journal of Manipulative and Physiological Therapeutics, Stage 3 hypertension patients experienced greater than 60 mmHg systolic blood pressure reduction after two to four weeks of fasting.

Follow-up data showed 100% retention through refeeding and 93% retention at six-week follow-up, with significant decreases in weight, BMI, total cholesterol, and LDL. Rooms run $209 to $599/night, with a typical 10-day stay costing approximately $3,385.

Buchinger Wilhelmi

Buchinger Wilhelmi has operated since 1953. That’s over seven decades of continuous medical fasting supervision. Their flagship on Lake Constance, Germany offers 10 to 28-night fasting programs starting from approximately $4,500 for 10 nights.

A 2019 observational study published in PLOS ONE followed 1,422 subjects through their protocol. 84.4% of those with pre-existing conditions reported improvement.

The study confirmed the protocol was safe across all age groups, including a 92-year-old man who completed 45 years of annual 21-day fasts.

SHA Wellness Clinic in Spain bridges the gap between luxury resort and medical facility better than anywhere else I’ve researched. Their Advanced Longevity program (7 nights from approximately $12,100) pairs diagnostics with functional medicine, ozone therapy, neuromodulation, and personalized nutrition.

Medical supervision is high, with physician-led assessments and a take-home protocol. SHA also has a Mexico location.

AIWO Retreats

AIWO is one of the most well-known names in the integrative retreat space, with locations globally. They combine wellness programming with structured health protocols and community experiences. Worth researching their specific location offerings since programming varies by destination.

Sensei Lanai, A Four Seasons Resort

Sensei Lanai offers the most premium luxury experience on this list. Guided Wellness stays from approximately $1,435/night include daily private sessions, nightly spa credit, and roundtrip flights from Honolulu through 2026.

Here’s what you’re actually paying for: world-class wellness coaching in one of the most beautiful settings on earth. Medical supervision is low, and you won’t get advanced diagnostics. But you’ll get an incredible experience.

Best International Longevity Destinations

International retreats offer either dramatically better value per dollar (Southeast Asia) or prestige and institutional depth (Europe).

Kamalaya Koh Samui

Kamalaya in Thailand is the best value on this list. A seven-night detox program runs approximately $4,000 to $5,500 including room, treatments, diagnostics, and meals. That’s 3 to 5x more treatment per dollar than comparable US retreats.

Their Longevity House adds binaural sound healing. They also offer:

  • IV therapy
  • Ozone therapy
  • Hyperbaric oxygen
  • Functional health assessments

Chiva-Som

Chiva-Som in Hua Hin, Thailand has operated for 30+ years at approximately $770 to $1,100/night with three daily wellness meals and one treatment per night included. More wellness-focused than medical, with naturopathic consultants rather than physicians.

Rythmia

Rythmia is the outlier on this list, and it earns the spot. It sits in Guanacaste, Costa Rica, one of the world’s five Blue Zones, and it works on the psychological drivers of aging that no blood panel touches, like chronic stress and unresolved trauma.

Rythmia reports that 98% of its 22,499 guests self-report a “life-changing miracle” during their stay, and that 93.42% still feel it working six months later.

Those are the retreat’s own self-reported numbers, so weigh them as marketing. The independent signal is harder to wave off: it’s the #1 customer-rated healing center on Tripadvisor, with more than 2,800 five-star reviews.

The seven-night program is dense. A typical stay includes:

  • Four ayahuasca ceremonies under medical supervision
  • Daily breathwork, yoga, and meditation
  • Two Dead Sea hydrocolonic cleanses and a healing massage
  • Integration classes, plus talks from founder Gerard Powell and Michael Beckwith
  • 17 farm-to-table organic meals

What separates it from the field of unvetted jungle operators is the medical infrastructure. Rythmia calls itself the only medically licensed facility in the world authorized to serve ayahuasca as a core therapy, with staff physicians and a medical evaluation built into the stay. I interviewed their Chief Medical Officer, Dr. Jeff McNairy, on the podcast, and the screening and oversight stood out.

I haven’t gone through a program there myself, so I won’t pretend to review the experience firsthand. Plant medicine isn’t for everyone: it interacts with several medications (SSRIs especially) and isn’t a fit for some psychiatric conditions, so treat it as serious medical and psychological work. Should You Try a Stem Cell or Regenerative Retreat?

Stem cell therapies show real promise, and many people report meaningful improvements. The challenge is the enormous quality gap between clinics.

BioXcellerator in Medellin, Colombia is the most prominent stem cell retreat targeting longevity seekers. Standard protocols run $18,500 to $25,000. They use umbilical cord mesenchymal stem cells processed in a GMP-certified on-site lab and include 12 to 19 months of post-treatment tracking.

Their self-reported success rate is 80 to 90% positive outcomes. Important context: that data is self-reported, not independently validated. The FTC also sent them a warning letter regarding COVID-19 claims.

Before spending $25,000+ on stem cells, ask yourself:

  • Can I see published outcomes data from an independent source?
  • What’s the mechanism of action for my specific condition?
  • What happens if it doesn’t work?
  • Have I optimized fundamentals first, or am I using this as a shortcut?

The gap between the best clinics and the worst is enormous. Due diligence matters more here than anywhere else on this list.

What to Do Before & After a Retreat

A retreat can kickstart change. But if you go back to the same environment, the same habits, the same stress, it was an expensive vacation.

Before you go:

  • Check if your retreat provides baseline testing. Many medical retreats (Canyon Ranch, SHA, Next Health) include comprehensive diagnostics. If yours doesn’t, get your own before you go.
