It also doesn’t get harder in a straight line.

The worst-feeling stretch hits early, in the first two days.

Then something odd happens.

Plenty of people feel clearer and steadier right as the deepest work begins.

That’s the part nobody warns you about. Miss the map and you quit at the low point, a day before the payoff.

Here’s where this map comes from:

  • Decades of Russian clinical practice, from Nikolaev to Sergei Filonov, brought into English by Dr. August Dunning.
  • My own dry fasts. A full five days this year, and a three-day last year.
  • Almost no Western lab studies, and there never will be. No ethics board approves pulling water from healthy people for five days. So the evidence here is clinical, and I’ll flag where modern research lines up.

Dunning and the Russian doctors describe the fuel transition as a gradual gear change across the first two days rather than a switch.

And dry fasting compresses the whole thing.

Because you’re pulling water as well as food, the metabolic pressure is higher. The common rule of thumb: five days dry does the work of roughly ten days of water fasting.

Theo Lucier, who coaches this protocol, explains why the dry version hits differently.

“A different enzymatic pathway turns on,” he says. “You don’t lose muscle, you don’t get gluconeogenesis. Fat, scar tissue, tumors, and senescent cells get shredded for fuel.” He calls it aggressive autophagy on steroids.

Dry Fasting After 0 to 12 Hours: The Quiet Setup

For the first 12 hours, your body still runs on recent food plus stored glycogen (liver and muscle).

Insulin starts falling and drops sharply by about hour 12.

Dunning notes that autophagy, your cellular cleanup process, can begin within a few hours of your last meal. The housekeeping is already switching on before you feel anything. This is setup work.

The real therapeutic window comes later.

Energy stays normal and hunger is mild.

Dunning likes starting in the evening so these easy hours pass during sleep. You wake up already a third of the way in.

Dry Fasting After 12 to 24 Hours: The Hormone Shift

Between 12 and 24 hours, falling insulin flips a hormonal switch.

Growth hormone and anti-diuretic hormone climb, which protects lean muscle and steers your body toward fat. By the evening of day one, Dunning reports being about halfway to ketosis.

Modern endocrinology lines up with the direction here. Low insulin plus rising growth hormone is a well-documented early-fasting state. Your mouth may turn sticky as water conservation kicks in, and hunger often fades sooner than you’d expect.

You’re not in ketosis yet but you’re setting the stage for it.

Dry Fasting After 24 to 48 Hours: Glycogen Burnout & Ketosis

Between 24 and 48 hours, liver glycogen runs down and fat metabolism takes over.

This is usually the most uncomfortable stretch.

Dunning calls the first two days the admission price, and that’s a fair name for how they feel.

Expect:

  • Achy muscles
  • Headaches
  • Thinner patience
  • Less power

Blood ketones theoretically climb through this window, which controlled human fasting studies put in the same 24-to-48-hour range.

His advice is to sleep through the 48-hour transition when you can.

Your body isn’t built for hard training here, so do less and let the switch finish. Once glycogen is gone, the hardest part of the handoff is behind you.

Dry Fasting After 48 to 72 Hours: Metabolic Water & the Acidotic Crisis

With no water coming in, your body makes its own.

Fat breakdown releases three things at once: glycerol that becomes blood glucose, fatty acids that feed your mitochondria, and metabolic water.

Dunning says the body makes plenty of water this way, and the biochemistry backs it.

Burning fat yields water. This is how a healthy person goes days without drinking. You’re not just tolerating dehydration, you’re running a water supply built from your own fat.

Then comes the low point.

The first acidotic crisis is the trough of the early fast. Filonov places it between days one and three.

The practical day-three version lands around 60 to 72 hours, as your body commits fully to fat and the blood shifts slightly acidic.

This is the moment people quit, right before the payoff.

Power stays low. Walking is fine for lymph flow if you’re steady, but this isn’t a performance window.

Crossing the crisis is what opens the repair phase. Discomfort is the signal that the real work is about to start.

Dry Fasting After 72 to 96 Hours: The Day-4 Stem-Cell Wave

Day 4 is the payoff.

The stem-cell wave centers on roughly 72 to 96 hours, when your body shifts from breaking things down to building them back.

In Dunning’s model, protein kinase A and insulin both fall to near zero. Stem cells across the body’s 220 niches move from dormancy into repair. He points to Muse cells drawn from fat as a special population that helps rebuild blood vessels.

This is Dunning’s mechanism, grounded in the Russian clinical experience of what deep fasting does.

Modern research supports the direction, without having pinned the exact hour in a dry-fasting human. Fasting cycles trigger stem-cell regeneration in animal work from the Longo lab. And ketone bodies like BHB act as signaling molecules that calm inflammation and switch on repair genes.

The clinical tradition got to day 4 first. The lab is catching up to the why.

Theo Lucier puts the stakes plainly. “If you make it to day 4, you get a stem cell release,” he says. “Make it to day 6, you get a second one.” His warning is blunt: quit during a stem-cell release on a 5-day fast and you harm your stem-cell production.

That’s the real reason the day-4 window is the one to protect.

The worst stretch of a dry fast sits right before the best one.

Day three into day four can feel strange, clearer in some ways and wiped out in others. That mix is normal. Don’t break this window casually.

Dry Fasting After 96 to 108 Hours: Peak & the Safe-Exit Point

The wave crests through day four and into the start of day five.

This is also where deeper repair can surface Herxheimer-like reactions as old debris gets cleared: mild nausea, gurgling, broken sleep, a general detox feeling.

It’s not pleasant, and it’s the part of the fast everything else is built around.

Here’s the practical line.

If you can’t reach the full five days, somewhere around 108 hours is the earliest safe exit. There’s tremendous bioindividuality from one person to the next, so that’s not an exact number.

By then you’re 12 hours past the day-4 peak and about 90% of the way to target. You’ve banked the wave instead of breaking mid-repair.

One thing to be clear about.

Stopping early won’t overtly physically harm you. You won’t collapse for ending a day sooner.

What breaking mid-wave does is interrupt the stem-cell repair before it finishes, so you leave the main benefit on the table. It can also deplete your stem cell pool for months afterward.

That’s the whole reason 108 hours is the floor, and if possible, experts recommend going through the full fifth day.

Whenever you exit, the refeed still gets the same care.

Pro Tip: Set a phone alarm for hour 108. If you ever have to bail early, that’s about safe-exit line, and it means you likely already cleared the day-4 stem-cell wave.

Dry Fasting After 108 to 120 Hours: Finishing Five Days

The last stretch of a five-day dry fast runs 108 to 120 hours.

Dunning says day five brings fatigue, low blood sugar, a shifting pulse, a coated tongue, and sometimes voice or throat changes.

It comes with a sense that the main work is done. It’s a quiet finish rather than a dramatic one.

Recent Dunning material treats five days as a complete one-wave protocol with far less recovery cost than seven. That’s why five days is the target.

One full wave, a clean endpoint, and a recovery most healthy people can absorb.

Do less.

Skip driving if your sleep is poor or your head feels off. The hard part is mostly behind you.

Dry Fasting After 120 Hours: The Exit Is the Intervention

Shchennikov, one of the early Russian dry-fasting doctors, taught that coming out of the fast matters more than going in. Break it wrong and you can undo the hard-won progress and pile the weight back on.

Five days without water leaves magnesium, potassium, sodium, and fluid balance depleted. Your gut has just had the longest rest of your life.

So the exit is deliberate: electrolytes and fluids first, sipped slowly, never chugged. No added sugars or refined carbs for about five days.

They spike insulin and regrow the fat cells you just shrank.

Broths before solids, then light, well-chewed food once your appetite returns. That’s the simple physiology.

How Long Can You Dry Fast? 5, 7 & the 11-Day Ceiling

The real ladder is short: five days at home, seven as an advanced extended fast, and up to eleven only under supervision.

Filonov puts unsupervised prevention at five days and clinical fasts at seven to eleven.

No established protocol runs a patient past eleven.

Days 5 to 6 (120 to 144 hours): the extended fast

Past the five-day finish, the fast stops being one clean wave and becomes a higher-stress extension.

Dunning says seven days drives deeper autophagy and more clearance than five.

Day six is variable, the hardest day in some logs and easier in others. Sleep suffers, weakness rises, and judgment gets cloudy. Treat this as supervised rest instead of a toughness test.

Days 6 to 7 (144 to 168 hours): approaching the second crisis

Around 144 hours the body approaches a second crisis.

Filonov describes it as therapeutic rather than acidotic. It usually lands between days seven and twelve, with the strongest healing around day nine. A seven-day fast only reaches the doorway of that window.

That’s why seven days is a gateway to the longer clinical fasts rather than the destination.

Days 7 to 11 (168 hours and beyond): the supervised ceiling

Serious clinical cases run nine, ten, or eleven days, with day nine at the healing peak. Eleven is the ceiling in the Russian tradition, and the exit risk climbs the longer you go.

This is what supervised medical fasts target. Running one solo isn’t the idea.
